Requesting an Athlete Assessment *

By checking the box below, you will receive an email with time and day options for an assessment. The assessment fee is $25 per athlete, due upon arrival. After receiving our email, email us back to confirm your assessment day and time, (first to respond-first to reserve basis) we will reserve your spot and email you an electronic waiver form to fill out prior to attending the assessment. -Assessment - 1. Show up at least 10 minutes prior to your scheduled assessment time. Your designated coach will collect your assessment payment, provide you with a wavier form (if you are under 18 years old, a legal guardian must be present) and discuss prior training/sports experience and any prior injuries, etc. 2. Assessment will be 60 minutes from start to finish. We will look at overall balance, mobility, hand-eye coordination, foot speed, understanding of body mechanics, explosiveness, etc. (parents are allowed to be present during assessment) -Post-assessment- 1. Upon completion of the assessment, your coach will speak with the parent and/or athlete regarding the athlete’s strengths and weaknesses 2. Within 48 hours of the assessment being completed, we ask that the parent and/or athlete email us (by using the same email thread in the “pre-assessment” stage) regarding their thoughts on being a part of what apPrep has to offer for the athlete. (we are only seeking out committed athletes who have specific goals to achieve, we are not a "get as many in the door as we can" type of training facility) We will send a follow-up email with available training days/times that fit the athlete's specific needs. #GoTime
